With a unnerving humming sound, you make a thin cut around the skull and lift it up, pulling out the brain and leaving the creature's body behind to rot. However, the creature is not dead; in fact, its brain is altered to survive the elements outside its skull with all its normal senses, in spite of the lack of eyes or ears. However, without functioning lungs, mouth, hands, or legs, the removed brain cannot use any somatic or vocal components and may only employ purely mental actions (including psionics). It loses all its natural armor and any items it was wearing (reduced to just a head and face slot in this new form) and it shrinks down four sizes. It has no movement modes (at least not by default) and no Str or Dex score. When removed, the brain lands in the creature's space to be picked up.

The brain will persist without need to eat, drink, or breathe, is biologically immortal, and can survive the elements as well as its original body did. Communication via telepathy is still possible with the brain, and most place these brains in jars for safekeeping where they may be telepathically interrogated as needed.

Because the creature is not dead, they cannot be revived. They may return to normal if their brain is placed back inside their body, its wound healing and the creature waking up in 3d6 rounds. Without the brain, their limp body will die in 1d4 days, so a new body may need to be re-created for the creature.

Some psions purposefully do this to themselves, provided they have telepathy and a method of movement. Though the loss of many limbs and ability hurts, their new size and immortality is worth it to some.
